The Three Most Essential Pieces of Information on Your Insurance Card

When you call our office we will first identify if you are a returning patient or new, then ask for your insurance information. You will need your:

  • Group Number
  • ID Number
  • Plan Type*

*For Medicare card holders, instead of plan type you’ll be asked for which ‘parts’ of Medicare you have. Medicare offers four parts in total, A,B,C, and D. These four parts refer to the different divisions of the Medicare program. In addition, there are supplemental ‘plans’ that you can add on that pay the gaps of your coverage, such as F, G, K, L, M and N.

The Differences Between PPO and HMO Insurance Plans

There are several insurance plans you can sign up for, but the most common ones we see are PPO and HMO.


PPO stands for a preferred provider organization plan. This type of insurance plan allows you to choose any doctor or specialist within your coverage network without needing pre-approval or a referral order from another doctor. Your insurance provider will have a list of doctors you can go to in the area you live, but you are free to see a doctor outside your network as well. You may have to pay more money to go to a doctor or specialist outside your network, but the beauty of a PPO is you are not restricted from doing so.

A health maintenance organization (HMO) is a plan where you pick one primary care doctor to take care of your needs. Any other service or doctor you need to see will require a referral from your primary care doctor, or what we call at our office a PCP. The advantage of an HMO plan is that your PCP will take care of any paperwork needed to see one of the doctors at our office.

Changes Coming to Medicare Cards in 2018

In 2018 Medicare will be mailing out new cards with new ID numbers. Below is an example of what your new Medicare card will look like.
